- 97% of executives believe generative AI will be transformative for their company and their industry (Accenture, survey of 2,300 global leaders).
The gap between "AI will transform everything" and "we've actually shipped governed AI workflows" is where most organizations live. It's uncomfortable. And most consultancies would rather sell you a transformation roadmap than sit with you in that discomfort.
Why pilots fail
The failure mode isn't technical. AI models work. APIs are available. Agents can write code, generate content, and make decisions. The failure mode is organizational:
No behavioral constraints. AI agents operate without defined boundaries — no explicit rules about what they should and shouldn't do. When an agent hallucinates, overspends, or ships unreviewed code, there's no governance system to catch it.
No quality gates. Outputs go from agent to production without structured review. The "move fast" ethos that works for human developers creates chaos when applied to autonomous agents.
No lifecycle management. Initiatives start as experiments and stay that way. There's no proposal process, no review cadence, no way to track what's working and what isn't.
What governance actually looks like
Governance isn't bureaucracy. It's the structure that makes shipping possible.

Behavioral constraints — Agent roles are defined by what they do, not who they are. "Defaults to NEEDS WORK, requires evidence for approval" is a behavioral constraint. "You are an expert reviewer" is not.
